I, the undersigned, as the Parent or Legal Guardian of the Participant, give my consent for my child to participate in the STEM A Chip Electronics Workshop. I understand that the Workshop will involve hands-on activities, including the use of electronic components and tools. I acknowledge that I have been informed about the nature of these activities and the associated risks, including but not limited to the risk of injury.

I understand and accept that participation in the Workshop involves inherent risks, including but not limited to physical injury, property damage, or unforeseen incidents. I acknowledge that I am voluntarily assuming these risks on behalf of the Participant.

I hereby release and discharge the Organization, its officers, agents, employees, and volunteers from any and all claims, liabilities, or damages arising from the Participant’s involvement in the Workshop, whether occurring on-site or off-site.

 In the event of a medical emergency, I authorize medical personnel to administer necessary first aid or emergency care to the Participant. I understand that all reasonable efforts will be made to contact me before medical treatment is administered, except when immediate action is required to protect the health and safety of the Participant.

I, the undersigned, grant permission to STEM A Chip, its representatives, agents, and employees to record, photograph, videotape, and/or interview the Participant during the Workshop. I understand that these materials may be used for promotional, educational, or marketing purposes, including but not limited to websites, social media, print materials, newsletters, presentations, and advertisements. I waive any rights to review or approve the final content and acknowledge that I will not receive compensation for such use.

I give my consent for the Participant to participate in a voluntary survey as part of the STEM A Chip Electronics Workshop. The survey will collect feedback on the Participant’s experience, skills learned, and overall satisfaction with the Workshop. I understand that the Participant may choose not to participate or withdraw at any time without affecting their involvement in the Workshop. Any data collected will be anonymized and used solely for educational, evaluation, or improvement purposes.

I acknowledge that I am solely responsible for the transportation of the Participant to and from the STEM A Chip Electronics Workshop. I understand that the Organization does not provide transportation and agree to ensure that the Participant arrives and departs on time and in a safe manner.

This Agreement shall be governed by and construed in accordance with the laws of the State of Texas, USA, without regard to conflict of laws principles. If any provision of this Agreement is found to be invalid, illegal, or unenforceable, the remaining provisions shall remain in full force and effect. Any invalid or unenforceable provision shall be modified to the extent necessary to make it enforceable while preserving the original intent of the Agreement.

By signing this Liability and Event Waiver and Release Agreement, I confirm that I have read, understood, and voluntarily agree to all the terms and conditions outlined herein. I affirm that I am the Parent or Legal Guardian of the Participant and have the legal authority to provide this consent. My signature below signifies my full understanding and acceptance of the risks and responsibilities associated with the Participant’s involvement in the STEM A Chip Electronics Workshop.
